The Minnesota Wild Adult Hockey League has a few online forms that managers and individuals are required to complete before taking the ice. Please use the links below to visit each form page.

Player registration with USA Hockey is mandatory to play in the Minnesota Wild Adult Hockey League. Visit the USA Hockey website BEFORE your first league game to complete your registration. You will need to know your USA Hockey registration number to complete the Liability Waiver. USA Hockey membership is valid for one year (Sept. 1-Aug. 31).
Fall is the start of the new USA Hockey Season all players must have a current 2019-2020 membership

Step 1: If this is your first time using the system, you will need to create an NSC Event HQ household account. Get started at NSC Event HQ
The account owner must be 18 years of age or older. (Only need one account per family)
Step 2: After you have created your account, verified your email and set up your password.
Step 3: You can then add your children and or spouse as “participants”. if applicable
To complete a waiver
Log in to your NSC Event HQ account
Once logged in,
Step 1: Click on the “Customer Tools” link in the main menu bar and then select “Complete waiver”.
Step 2: Select the event. You may also filter the events by sport and/or name.
Step 3: Enter the Team id -provided by the team manager
Step 4: Select your name from the roster (if you are not on the roster contact the team manager)
Step 5: Select your name from the drop down list
complete the waiver entering your USA Hockey Number , Highest level of play and then accept
